What to know before your first visit
Booking a first visit
Call (616) 730-2600 or book online. In most instances we can see you within a few days, and on some occasions the same day.
Your forms, sent ahead
As a courtesy to you and the trees, we send documentation electronically wherever we can. Contact the office and we will send you the right forms. If you would rather complete them here, please arrive fifteen minutes early so we have time to register you properly.
What to bring
Your dental and medical insurance cards, and a list of all the medications you currently take. Take your usual medication on the day of the visit, including any antibiotics your doctor requires before dental appointments.
Patients under 18
Anyone eighteen or younger needs to be accompanied by a parent or legal guardian.
If your plans change
As a courtesy to other patients and to our team, please call the office as soon as you know you cannot keep an appointment, or that you are going to be late.
